Sample Letter
This letter is used to formally request that your child's school conduct an educational evaluation when your child is struggling, and you suspect they may have a disability or need additional support.
In the State of Montana, once the school receives your signed consent for the evaluation, they generally have 60 days
to complete the evaluation.
